Good day, I would like to log a formal complaint. I previously contacted one of your consultants at Massmart to request pre authorization for a 4D maternity scan. I was clearly informed that no pre authorization is required for this type of claim, and that I could go to any 4D scan provider, pay for the scan, and then submit the claim to be reimbursed.
At no point was I advised that a practice number was required. I went to Baby scan and wellness Centre for the 4D scan, and they confirmed afterwards that they only have a registration number, not a practice number.
Had I been informed upfront that a practice number was required, I would have gone to a different provider. Because of the incorrect information provided to me by the consultant, I went ahead with the scan in good faith. I have now submitted the claim and proof of payment, and I request that this matter be reviewed so that I may be reimbursed. Thank you
